Wayside Recovery Center is looking to hire a full-time Monday-Friday Children's Case Manager at our Family Treatment Center in Minneapolis, MN.

This role will be responsible for providing case management, advocacy services and parenting services with primary focus on the child and secondary focus on mother and family relationship at Wayside Family Treatment, a Statute 245G substance use disorder treatment program for women and their children. This position will be part of the Family Services team; a team designed to wrap family centered and culturally responsive care around diverse family units.

JOB RESPONSIBILITIES:

Duties including, but not limited to, the following:

· Provide family services case management including parent and family case management, education groups, individual sessions, child and parent assessments, early childhood development screenings, and coordination of care.

· Facilitate culturally specific parenting classes for mothers

· Assist clients in setting goals based on their individual and family needs assessments, and in identifying steps to achieve their goals; monitor progress.

· Identify high-quality community providers and agencies whose services are relevant to client needs. Provide outreach services to engage and connect clients with the community agencies that will help meet their needs and goals.

· Coordinate scheduling and attend case staffing and appointments with community providers with whom client is involved. Maintain regular contact with client’s network of service providers

· Make appropriate referrals and coordinate care to assist client in obtaining resources and meeting treatment planning goals for client and their family.

· Assist client in setting up medical providers and appointments for their children when needed

· Provide transportation to client/children appointments and to obtain resources as needed.

· Coordinate daycare and school enrollment for children and assist client with completing necessary paperwork for daycare/school

· Maintain accurate, timely and complete records, including but not limited to documenting progress, screenings, collateral contacts, and assessments.

· Maintain on-going communication with multi-disciplinary team and collateral workers, including providing written reports and discharge information.

· Complete all required child development assessments and enter the data in the specified database.

· Role model behavior that is consistent with and supportive of people in recovery from substance use disorder. Such behavior will include upholding

ethical standards of Wayside Family Treatment programs, reliable attendance, mediation and/or conflict resolution, respectful assertiveness, and open communication. Demonstrate respect and appreciation for diverse cultures and lifestyles.

· Collect and record data in a timely manner- as assigned for grant purposes

· Remain in contact with clients post discharge for up to two years and collect and record data as needed.

· Must meet regularly with each mother and their children to help identify needs, problem solve, set goals, and work on action plans around health, parenting, education, and employment.

· Offer groups for children of various ages using sequential, research-based violence prevention curricula to help assets that reduce their risk of addiction and violence.

· Offer recreational events that keep the focus on sobriety and self-improvement.

· Offer Parenting Support and Advocacy through referrals to outside early childhood programs, development assessments, and therapy services.

· Create Partnerships with Early Childhood Family Education programs for families with toddlers.

· Create weekly events such as family game nights, movies, barbecues, crafts, and sports tournaments further build relationships between mothers and their children.

Additional Duties:

· Assist in parenting and family program planning, implementation, and evaluation.

· Work as part of a multi-disciplinary team, including participation in weekly team meetings

· Participate in in-services, workshops and other continuing education relative to quality client care.

· Follow all Statute 245G and individual licensing requirements and standards, including, following confidentiality laws and mandated reporting.

· Provide crisis intervention/evaluation as needed.

· Participate in weekly supervision

· Represent Wayside Family Treatment in the community in a positive and constructive manner.

· Perform additional duties as assigned.

QUALIFICATIONS:

Required

· Bachelor’s degree in behavioral sciences

· Must be able to pass DHS background study and be free of communicable disease.

· Experience in parent education and family advocacy

· Demonstrated ability to work with and relate effectively to women, children, and staff of diverse economic and cultural backgrounds.

· Demonstrated experience working effectively with women and children that identify Black, Indigenous or Persons of color is highly preferred.

· Must have current MN driver’s license and good driving record.

· Must have access to reliable transportation.

· No DWIs in last three years; No more than three moving violations/accidents in past three years

· Must be eighteen years or older.

· Strong attention to detail and flexibility; thinking capabilities and humor are welcome.

· Licensed Social Worker

· Previous experience and/or understanding of substance use disorder and recovery

Physical Requirements: Must be able to lift 25 lbs.

Must be able to climb stairs.
